Affordable Therapy Program

At Cultura Health, we’re proud to offer low-cost, affordable individual and relationship counseling. With our Affordable Care Program, you don’t need insurance and you don’t need to be in-network. We provide professional counseling that fits your budget — our affordable rates mean top-notch authentic mental health care for all.

How are fees determined?

We use a sliding fee scale and offer rates that are discounted from our usual and customary rates, as determined by the recipient’s ability to pay. Depending on the recipient’s family size and household income, rates for services are discounted, as shown in this table. View our Sliding Fee Scale effective as of August 8, 2023.

What is Cultura Health’s Sliding Fee Based On?

Our sliding fee is based on federal poverty guidelines and is subject to change.

What if my household is bigger than 10 members?

If your household consists of more than 10 people, you need to schedule an appointment with a Cultura Health Financial Advocate (MFA) to calculate the fee that would apply to you. To schedule an appointment please call 832.553.6225.

When is the fee due?

The fee is due at the time of visit. If you are unable to pay your bill at the time of the visit, you will need to meet with a financial counselor.


Sliding Fee Scale Requirements

Proof of Identity

    • Any type of identification with your picture and name: such as a driver’s license, state-issued ID, school ID or work ID. Documents such as birth certificates, school documents, and immigration applications are needed to establish dependent qualifications. Dependents age 21 and younger qualify as part of a household.

Proof of Address

    • Examples include utility bills (cable bill, cell phone bill, etc.), a copy of a lease agreement voter registration card automobile insurance documents or any piece of  postmarked mail you have received recently.

Proof of Income 

    • Recent proof (dated within the last 45 days) of all household income including earned and unearned income. This would also include income for your spouse and any minor children for the past 30 days.

Providing Proof of Income

    1. Earned Income
       is paid through employment. Most earned income will be countable by pay stubs provided through your employer. Cash or self-employment income can also be counted with the appropriate documentation. 
    • Unearned income
       is provided through financial support (usually a family member or friend), legal judgments (like child support), retirement, investments, social security, disability payments, etc.
